Through programs such as the Community Choices Waiver, Long Term-Personal Care Services, and more, we help clients secure in-home care assistance, therapy, nursing services, medical equipment, home modifications, respite for caregivers, and case management support. Each program is designed to help individuals maintain their independence and improve quality of life, without the need for institutional care. Whether it’s assistance with daily living activities, navigating eligibility requirements, or accessing specialized housing support, our team is here to help every step of the way.

Supporting Louisiana, One Step at a Time
While receiving services through our agency, we ask for your cooperation in the following areas, as required by Medicaid of Louisiana:
Arrange/coordinate for the specific services as specified on the individualized written plan of care.
Comply with the provider’s written procedures for obtaining services and support the provider’s need for timely communication, such as returning calls or canceling scheduled visits in advance.
Have full access to all approved services provided by the waiver service provider.
Notify L & D of any changes in appointment times, dates, service locations, or ongoing service needs, as well as any changes in the client’s health.
Follow the individual service plan to ensure that the approved service hours are not exceeded on a daily or weekly basis.
Inform L & D of the client’s specific preferences, such as favorite foods, preferred bathing methods, recreational activities, or any special care routines.
If at any point the client requires additional services to support their health or overall well-being, L & D will assist in identifying and connecting you with the most appropriate resources available.
