Community Choices Waiver – Personal Assistance Services (CCW-PAS)
The Community Choices Waiver (CCW) is designed to help eligible individuals remain in their homes and communities for as long as possible with personalized, flexible support. It’s intended for adults who meet Medicaid’s nursing home level-of-care criteria but want to avoid institutional placement. CCW offers a wide range of services tailored to maintain independence and quality of life. These include Personal Assistance Services for daily living tasks like bathing and dressing, Support Coordination to guide and organize care, Adult Day Health Care, Nursing Services, Therapy Services, Home-Delivered Meals, and short-term respite for caregivers. The program also offers assistive technology and medical supplies, home modifications such as ramps or grab bars, personal emergency response systems, and even help with securing permanent supportive housing. Eligibility is determined by age (21+), Medicaid status, and an assessment showing nursing home-level care needs. By offering these supports in the community, CCW helps people avoid institutional settings and continue living with dignity and choice at home.
