Through partnerships with VA programs and community-based supports, including the VA Community Care Network (CCN), we help veterans access the timely, high-quality care they need to live safely and independently at home. The CCN connects veterans with private healthcare providers when VA facilities are unable to deliver the required care due to capacity, location, or specialized needs, ensuring seamless access to essential services. Eligible veterans may qualify for in-home assistance with daily living activities, respite care for family caregivers, homemaker and home health aide services, and support through programs like VA Aid & Attendance and Veteran-Directed Care. These services promote dignity, honor veterans’ choices, and reduce the need for institutional care whenever possible. Our team works closely with veterans, their families, and local VA offices to verify eligibility, coordinate authorizations, and develop personalized care plans that respect each veteran’s unique needs and goals.
